Please be advised that, pursuant to Rule 2:5-1(f)(1), I have approved
the attached revised version of the Notice of Appeal to
the Appellate Division form, which appears as Appendix IV to the Rules
Governing the Courts of the State of New Jersey. This revised version
supersedes the previous version effective as of the date of this
notice.
The revisions consist of two additions to the text of the form:
(1) to add language on the first page to the effect that, in consolidated
actions, all issues as to all parties in all actions must have been disposed of
before the matter can be appealed as of right, and (2) to add language on the
second page so as to request a list of the dates of the trial or hearing if the
transcript is in the possession of the attorney or the pro se litigant.
