It is ORDERED that the attached revisions to Appendix XII-A ("Summons") of the Rules Governing the Courts of the State of New Jersey are adopted, to be effective immediately. The revisions effected by this order are only to Side 2 of the Summons; there are no revisions to Side 1 of the Summons.
